 Dear Client,

 It is with mixed emotions that I am writing to let you know that I will be closing my practice as of March 1, 2019, due to unanticipated family health issues that will necessitate my relocation.  I have truly enjoyed my role as your therapist, and the friendships we have forged over the years, and I thank you for the privilege of providing care to you or your family member. 

Please arrange to have another psychotherapist provide treatment for you prior to the noted closure date. I will be happy to provide a list of local therapists for you to select a new provider from.  

  I will gradually cut back on the number of hours I will be practicing and will no longer be seeing patients on Mondays or Fridays in the New Year.  Effective March 1, I will be closing all client files. I am attaching a Release of Information Authorization form and will be happy to forward a treatment summary to your new provider, on receipt of the signed document, prior to March 1, 2019.

It has been a pleasure and an honor to work with you, and I wish you all the best for your future health and happiness.
